Please print out and complete the forms at the bottom of this page prior to using Prairie Pals’ pet services:

Client Information Form - please fill out one per family.
Pet Information Form - please fill out one for each pet that will be in our care.
Vet Authorization Form - please fill out and sign one per family.
Client Agreement - please read completely and sign one per family.
*Current Visit Form - please fill out one for each pet that will be in our care.

*You will need to fill out this form each time your pets are in our care.

In addition, please provide a copy of your dog’s most current vaccination records to keep on file. Rabies vaccinations are required for all pets. For stay at the Pet Inn, DHPP and Bordetella are also required. All dogs over 1 year that are staying at the Pet Inn are required to be spayed or neutered as well.
